Job Summary
We are seeking a highly skilled and detail-oriented Registered Nurse to join our clinical support team at our Mohali, India office. This is a non-patient-facing, non-bedside role focused on clinical documentation auditing, chart review, and remote communication with U.S.-based Registered Nurses and patients located in California.
The incumbent will play a critical role in ensuring the accuracy, completeness, and regulatory compliance of nursing documentation, while also serving as a communication bridge between clinicians and patients via phone calls. This position requires strong clinical judgment, excellent documentation skills, and a thorough understanding of home health standards, quality assurance, and U.S. healthcare compliance expectations.
Key Responsibilities
Clinical Documentation Auditing & Quality Review
- Review, audit, and validate nursing notes, visit documentation, and electronic health records (EHR/EMR) completed by U.S.-based Registered Nurses.
- Ensure all charting meets California state regulations, Medicare/Medicaid (CMS) guidelines, agency policies, and accreditation standards.
- Identify documentation gaps, inconsistencies, or compliance risks and provide clear feedback to clinicians for correction.
- Monitor timeliness, accuracy, and completeness of clinical documentation, including visit notes, care plans, and follow-up records.
- Support quality improvement initiatives by identifying trends, recurring documentation issues, and opportunities for staff education.
Communication & Care Coordination (Remote)
- Conduct professional phone calls with patients after clinician visits to verify that documented services accurately reflect the care provided.
- Communicate with U.S.-based clinicians to clarify charting, confirm visit details, and resolve discrepancies in documentation.
- Serve as a liaison between patients, clinicians, and supervisory staff to ensure clear, respectful, and timely communication.
- Escalate clinical or documentation concerns to appropriate supervisors in a timely and professional manner.
Compliance & Administrative Support
- Maintain strict confidentiality and compliance with HIPAA and organizational privacy standards.
- Accurately document audit findings, follow-ups, and communication outcomes within the EHR or internal systems.
- Assist with internal audits, regulatory reviews, and preparation for external surveys or inspections.
- Adhere to established workflows, turnaround times, and quality benchmarks set by the organization.
Qualifications & Requirements
Education & Licensure
- Bachelor’s or Master's degree in Nursing (B.Sc. & M.Sc Nursing) or equivalent.
- Valid and active Registered Nurse (RN) license (India or internationally recognized).
Experience
- Minimum 10–15 years of Registered Nurse experience, preferably in home health, community health, clinical documentation, utilization review, or quality assurance.
- Prior experience with U.S. healthcare systems, home health agencies, or California-based clinical operations is strongly preferred.
- Hands-on experience with EHR/EMR systems and clinical documentation standards.
Skills & Competencies
- Strong knowledge of nursing documentation standards and clinical workflows.
- Exceptional attention to detail and analytical skills.
- Excellent verbal and written English communication skills (required for U.S. patient and clinician interaction).
- Professional phone etiquette and patient-centered communication approach.
- Ability to work independently while collaborating effectively with remote, cross-border teams.
- Proficiency in MS Office and healthcare documentation platforms.
Work Schedule
- Full-time, on-site position based in Mohali, India.
- Work hours aligned with U.S. (California/Pacific Time) operations, including evening or night shifts as required.
What This Role Is Not require
- This role does not involve physical patient assessments, bedside nursing, or on-site patient visits.
- All patient and clinician interactions are conducted remotely via phone and electronic systems.
