Daily rounds and evaluation of the social and emotional needs of patients undergoing treatment
· Ensure the analysis of the feedback and interact with the patient on real time and understand the reason for unsatisfactory feedback maintain the record of all the responses and focus on the issue resolution or corrective actions in response to patient complaints/issues/queries.
· Provide counselling and support to patients and their families
· Helping patients understand their diagnosis, medical conditions and treatment plans and accompanying the clinical staff in the orient the patient to the facility
· Assisting patients and families with the emotional impacts of illness and hospitalization
· Providing information and referrals to community resources, such as financial assistance, transportation, housing, psychological and access to support groups
· Collaborating with healthcare providers, support staff, and other professionals to advocate for the patient’s needs
· As needed be a part of cancer awareness programs for the spread of cancer awareness
· To coordinate with the nurse in charges and supervisors and clinical team for smooth stay of the patient and call for intervention in case of any complaints received.
· Assist patients and families with paperwork related to insurance and medical programs
· Conduct patient engagement programs
· Advocate for patient rights and needs
· Assist the patient from the time of admission through discharge and record the report on patients’ status
· Play a crucial role in discharge planning, ensuring that patients have the necessary support and resources when they leave the hospital and transition to post-hospital care.
· Arranging post-discharge services and resources, such as home healthcare, rehabilitation services, and durable medical equipment
Pay: ₹15,000.00 - ₹20,000.00 per month
