Clinical Pharmacist – Hospital Side
Qualification:
- Doctor of Pharmacy (Pharm.D)
- Pharm.D graduates with hospital/clinical pharmacy exposure preferred.
- Registered with the applicable State Pharmacy Council, wherever required.
Preferred Experience:
- 2+ years –clinical rotation experience can be considered.
- Experience in hospital pharmacy, ICU, PICU/NICU, paediatrics, OBG, or multispecialty hospitals is an advantage.
Key Hospital Responsibilities
- Participate in clinical rounds with doctors and healthcare teams.
- Review medication orders for dose, frequency, route, interactions and contraindications.
- Perform medication reconciliation during admission and discharge.
- Monitor and report Adverse Drug Reactions (ADR).
- Identify and prevent medication errors.
- Provide drug information to doctors and nursing staff.
- Support Antimicrobial Stewardship Program (ASP).
- Monitor high-alert and high-risk medications.
- Provide medication counselling to patients and caregivers.
- Monitor appropriate use of antibiotics and other critical medicines.
- Review drug therapy and provide evidence-based recommendations to clinicians.
- Maintain clinical pharmacy documentation and intervention records.
- Participate in pharmacy/medication safety audits and quality improvement activities.
- Support compliance with NABH and hospital medication-management standards.
Pay: ₹13,047.83 - ₹29,875.27 per month
