Hiring: Case Manager – Social Work
Abhasa Rehab & Wellness is looking for a compassionate, responsible, and committed Case Manager to join our team.
Location: Thondamuthur
Qualification:
- M.S.W. (Medical & Psychiatry)
- Experience in therapeutic/rehabilitation settings is preferred.
About the Role:
The Case Manager will coordinate between clients, families, clinical teams, and center operations, ensuring smooth client care and effective communication throughout the recovery journey.
Key Responsibilities:
- Coordinate client admission, onboarding, and discharge processes
- Maintain regular communication with clients and families
- Coordinate family calls, family sessions, and progress updates
- Monitor therapeutic activities and counselling interventions
- Coordinate with doctors and clinical teams
- Review case documentation, reports, and incident records
- Support and guide the counselling and support teams
- Ensure adherence to centre SOPs and quality care standards
We are looking for someone with strong communication, leadership, empathy, and coordination skills, who is willing to make a long-term commitment to the organisation.

Pay: ₹50,000.00 - ₹60,000.00 per month
