Position: MRD Executive / Medical Records Executive
Department: Medical Records Department (MRD)
Location: Atlantis Healthcity, T. Nagar, Chennai
Job Summary
We are looking for a detail-oriented and organized MRD Executive to manage and maintain patient medical records accurately and securely. The candidate will be responsible for medical record documentation, filing, retrieval, data entry, discharge records, and coordination with various hospital departments while ensuring confidentiality and compliance with hospital policies.
Key Responsibilities
Medical Records Management
- Collect, verify, organize, and maintain inpatient and outpatient medical records.
- Ensure patient files contain complete and accurate clinical documentation.
- Arrange and file medical records systematically for easy retrieval.
- Retrieve patient records whenever requested by authorized departments or healthcare professionals.
- Track movement of physical files and ensure timely return to the MRD.
Discharge Documentation
- Coordinate with nursing, billing, doctors, and other departments for completion of discharge records.
- Verify discharge summaries, investigation reports, prescriptions, consent forms, operative notes, and other required documents.
- Identify missing or incomplete documentation and coordinate with the concerned department for correction.
Data Entry & Hospital Systems
- Enter and update patient information accurately in the hospital information system (HIS/HMS).
- Maintain electronic medical records and ensure proper scanning/uploading of documents.
- Support document indexing and digital record management.
- Maintain accurate records of admissions, discharges, transfers, and other relevant patient information.
Record Quality & Compliance
- Check medical records for completeness, accuracy, and proper documentation.
- Follow hospital SOPs, patient confidentiality requirements, and applicable medical-record management standards.
- Ensure records are accessed only by authorized personnel.
- Assist during internal and external audits by providing required documentation.
Coordination
- Coordinate with OPD, IPD, Emergency, Nursing, Billing, Insurance/TPA, Laboratory, Radiology, Pharmacy, and Medical Departments for documentation requirements.
- Respond to authorized requests for medical records in a timely manner.
- Assist doctors and other healthcare professionals in locating required patient records.
Medical Record Retrieval & Reporting
- Maintain proper records of issued and returned files.
- Prepare departmental reports and statistics related to medical records.
- Assist in maintaining admission, discharge, mortality, and other hospital statistics as required.
- Support the preparation of records for medico-legal cases and insurance/TPA requirements.
Requirements
- Any Graduate / Diploma in Medical Records Administration, Health Information Management, Hospital Administration, or a related field.
- 1–3 years of experience in a hospital MRD/Medical Records Department preferred.
- Experience in a multi-specialty hospital will be an added advantage.
- Knowledge of HIS/HMS, MS Office, electronic medical records, and medical documentation.
- Good typing and data-entry skills.
- Strong attention to detail and accuracy.
- Good communication and interpersonal skills.
- Ability to maintain strict patient confidentiality.
- Ability to work independently as well as with a multidisciplinary hospital team.
Preferred Skills
- Knowledge of medical terminology.
- Understanding of hospital documentation and discharge procedures.
- Knowledge of insurance/TPA documentation will be an advantage.
- Ability to identify missing or incomplete medical records.
- Strong organizational and file-management skills.
- Ability to handle sensitive patient information professionally.
- Willingness to work in rotational shifts, if required.
