Wellness Coordinator (Remote – India)
Job Summary:
We are seeking compassionate and proactive Wellness Coordinators to support integrated care management for our US-based healthcare clients. In this role, you will remotely engage with patients—including those with chronic health conditions—to promote wellness, provide emotional and behavioral support, and assist in coordinating care plans as part of a multidisciplinary care team.
Key Responsibilities:
- Interact with US-based patients via phone or digital platforms to support wellness and care management programs
- Support and engage patients with chronic conditions to encourage adherence to care plans and healthy behaviors
- Provide empathetic communication, motivation, and behavioral support to patients
- Monitor patient progress and document interactions accurately in EHR (Electronic Health Record) systems
- Maintain patient records and follow care management protocols
- Collaborate with healthcare teams to ensure continuity of care
Required Qualifications:
- Master’s degree in Psychology (mandatory)
- Experience working with EHR (Electronic Health Record) systems
- Prior experience supporting remote or international patients, preferably US-based
- Comfortable working night shifts aligned with US time zones
- Strong communication, listening, and documentation skills
- Experience in telehealth, behavioral health support, chronic care management, or care coordination preferred

Pay: ₹20,000.00 - ₹40,000.00 per month
