Job Summary
We are looking for an experienced AR Analyst specializing in US healthcare surgery and anesthesia billing. The candidate will be responsible for following up on outstanding insurance claims, identifying the causes of non-payment or underpayment, resolving denials, and ensuring timely reimbursement from insurance payers.
The ideal candidate should have strong knowledge of surgical and anesthesia claims, payer guidelines, modifiers, anesthesia time calculations, denial handling, appeals, and insurance follow-up.
Key Responsibilities
- Follow up on outstanding insurance AR through payer calls, portals, websites, and written correspondence.
- Review and work surgery and anesthesia claims across different AR aging buckets.
- Identify the root causes of unpaid, underpaid, rejected, or denied claims.
- Verify claim status, payment details, denial reasons, and expected resolution timelines.
- Take appropriate action on claims involving eligibility, authorization, coding, medical necessity, bundling, credentialing, timely filing, and documentation issues.
- Prepare corrected claims, reconsideration requests, and payer appeals with supporting documentation.
- Review EOBs, ERAs, correspondence, and payer responses to determine the next course of action.
- Identify contractual underpayments and escalate them for further review.
- Coordinate with coding, charge entry, payment posting, denial management, and credentialing teams.
- Maintain complete and accurate follow-up notes in the billing system.
- Prioritize high-value and aged claims to improve collections and reduce AR days.
- Follow HIPAA, payer, client, and organizational compliance requirements.
- Meet assigned productivity, quality, collection, and turnaround-time targets.
- Escalate recurring payer issues and denial trends to the team lead or management.
Surgery Billing Knowledge
The candidate should have working knowledge of:
- Professional and facility surgery claims.
- Global surgical package rules.
- Preoperative, intraoperative, and postoperative billing.
- Multiple, bilateral, assistant-at-surgery, discontinued, staged, and repeat procedures.
- Common surgical modifiers, including 24, 25, 50, 51, 52, 53, 54, 55, 56, 57, 58, 59, 62, 66, 78 and 79.
- Bundled procedures, NCCI edits, medical necessity, and authorization requirements.
- Diagnosis-to-procedure compatibility and supporting medical documentation.
Anesthesia Billing Knowledge
The candidate should understand:
- Anesthesia base units, time units, and payer conversion factors.
- Anesthesia start and stop time documentation.
- Medical direction, medical supervision, and personally performed anesthesia services.
- Anesthesia modifiers such as AA, AD, QK, QX, QY and QZ.
- Physical-status modifiers P1–P6.
- Qualifying circumstances and monitored anesthesia care requirements.
- Anesthesiologist and CRNA billing scenarios.
- Concurrency, documentation, and payer-specific reimbursement guidelines.
- Common anesthesia denials involving time, modifiers, authorization, medical necessity, and provider credentialing.
Required Qualifications
- Minimum 2 years of hands-on experience in US healthcare AR follow-up.
- Experience handling surgery and anesthesia billing accounts is required.
- Strong understanding of the complete US healthcare revenue cycle.
- Knowledge of Medicare, Medicaid, commercial insurance, and managed-care payer processes.
- Familiarity with CPT, ICD-10-CM, HCPCS codes, modifiers, CMS-1500 claims, EOBs, and ERAs.
- Experience in denial analysis, corrected claims, reconsiderations, and appeals.
- Ability to communicate effectively with US insurance representatives.
- Good analytical, problem-solving, and account-documentation skills.
- Strong written and verbal English communication.
- Ability to work independently and manage assigned AR inventories.
- Willingness to work during US business hours.
Preferred Qualifications
- Graduate in any discipline.
- Experience with physician, ambulatory surgery center, or hospital-based anesthesia billing.
- Familiarity with major US payer portals and medical billing software.
- Experience in payment variance and underpayment identification.
- Knowledge of payer contracts and reimbursement methodologies.
- Strong Excel and reporting skills.
