The Medical Documentation Specialist (MDS) is responsible for delivering accurate, timely, and comprehensive clinical documentation that supports healthcare professionals in providing high-quality patient care. Working as an extension of healthcare professionals, the MDS documents patient encounters within Electronic Health Record (EHR) systems while ensuring compliance with organizational quality standards, provider preferences, and applicable healthcare regulations.
Medical Documentation Specialists leverage Commure's Ambient AI technology and documentation workflows to reduce providers' administrative burden, enabling providers to spend more time focusing on patient care.
This is an entry-level opportunity designed for recent graduates and early-career professionals. Every Medical Documentation Specialist begins their journey with Commure's structured Training and Development Program, designed specifically for recent graduates and early-career professionals. The program provides a strong foundation through a combination of instructor-led learning, hands-on practice, real-world simulations, regular assessments, continuous coaching, and personalized feedback, enabling participants to build the knowledge, skills, and confidence required to succeed in the role.

The Medical Documentation Specialist is accountable for:
Delivering accurate, complete, and timely clinical documentation that reflects healthcare professionals' intent.
Supporting providers by reducing documentation burden and improving workflow efficiency.
Maintaining high standards of documentation quality, compliance, and confidentiality.
Learning and applying clinician-specific documentation preferences, specialty workflows, and documentation standards through structured training.
Continuously building knowledge and proficiency while contributing to operational excellence within Ambient Operations.
Clinical Documentation
Accurately document patient encounters within Electronic Health Record (EHR) systems.
Capture patient history, review of systems, physical examination findings, assessments, diagnoses, treatment plans, procedures, medications, and other clinically relevant information.
Ensure documentation accurately reflects the clinician's observations, clinical assessments, and medical decision-making.
Review completed documentation for completeness, consistency, and accuracy prior to submission.
Complete documentation within established turnaround time and productivity expectations.
Apply learnings from onboarding, coaching sessions, and quality feedback to continuously improve documentation accuracy and efficiency.
Clinician Support
Provide documentation support for clinicians across multiple specialties including, but not limited to:
Learn clinician-specific workflows, documentation preferences, and specialty requirements through structured training and practical experience.
Adapt documentation style and workflow to individual clinician requirements.
Communicate professionally and effectively while supporting providers and collaborating with internal stakeholders.
Documentation Quality
Produce documentation that meets established organizational quality standards and clinician expectations.
Learn and apply specialty/provider-specific documentation guidelines and best practices.
Identify documentation inconsistencies and seek clarification through appropriate channels when required.
Incorporate feedback from quality reviews, coaching sessions, and audits into daily practice.
Participate in quality improvement initiatives and calibration activities.
Technology & Workflow Management
Utilize Electronic Health Record (EHR) systems and documentation platforms effectively.

Maintain compliance with HIPAA and all applicable patient privacy, confidentiality, and information security requirements.
