Social Worker Job Description
- Conduct intake assessments and maintain individual case files for residents and trainees.
- Provide psycho-social counselling to persons with disabilities and their families.
- Develop individualized rehabilitation and empowerment plans.
- Monitor progress and update care plans periodically.
- Assist beneficiaries in accessing medical, educational, and livelihood services.
- Facilitate linkages with government schemes, social welfare departments, and community resources.
- Organize family meetings and provide guidance for reintegration and long-term care.
- Conduct community outreach programs to raise awareness on disability inclusion, accessibility, and rights.
- Support admission and outreach campaigns to identify new beneficiaries.
- Represent the Centre in community and stakeholder meetings.
- Work closely with physiotherapists, trainers, caregivers, and psychologists to ensure holistic care. Support program events, workshops, and training sessions.
- Participate in staff meetings and professional development activities.
- Collect, compile, and maintain data related to admissions, training attendance, program outcomes, and beneficiaries.
- Develop and manage databases or spreadsheets for program monitoring and evaluation.
- Coordinate with vocational trainers, social workers, and administrative staff for timely data submission.
