Job Summary
Provide skilled nursing care to patients at home by conducting assessments, developing care plans, delivering clinical care, supervising caregivers, maintaining documentation, and ensuring quality patient outcomes.
Key Responsibilities
- Conduct home nursing visits and patient assessments.
- Develop and implement individualized care plans.
- Maintain accurate clinical records and update the CET Application.
- Supervise caregivers and monitor quality of care.
- Coordinate with Customer Care, Logistics, and Finance teams.
- Conduct patient follow-up calls and attend operational meetings.
- Train caregivers and support competency development
- Submit daily activity logs, supervision reports, and required documentation.
- Participate in quality improvement initiatives and monthly review meetings.
Qualifications
- B.Sc. Nursing or GNM
- Valid Indian Nursing Council (INC) Registration
- Valid Driving License
- Minimum 1 year of clinical nursing experience
