Medical Transcriptionist — Job Description
A Medical Transcriptionist is responsible for converting physicians’ and other healthcare professionals’ dictated clinical information into accurate, properly formatted medical documents while maintaining patient confidentiality.
Key Responsibilities
- Transcribe medical reports, clinical notes, discharge summaries, consultation notes, operative reports, and other healthcare documents.
- Listen to recorded dictations and accurately convert them into written reports.
- Review and proofread transcribed documents for grammar, spelling, medical terminology, and accuracy.
- Ensure correct formatting and documentation according to hospital standards.
- Verify patient names, medical terms, diagnoses, medications, procedures, and other clinical information.
- Maintain accuracy and consistency in medical records.
- Use medical terminology, abbreviations, anatomy, pharmacology, and disease-related terminology appropriately.
- Coordinate with doctors or relevant departments to clarify unclear or incomplete information.
- Maintain strict confidentiality and security of patient information.
- Complete transcription assignments within required turnaround times.
- Enter and update information in electronic medical record (EMR/HIS) systems.
- Follow hospital policies, documentation standards, and applicable data-protection requirements.
