Job Description * Assess patients' physical, emotional, and psychosocial needs and develop individualized care plans.
- Provide nursing care, including medication administration, wound care, catheter care, and symptom management.
- Monitor and manage pain and other distressing symptoms to improve patient comfort and quality of life.
- Assist patients with activities of daily living and promote independence wherever possible.
- Educate patients and family members on disease management, medication use, nutrition, and home care practices.
- Provide emotional support and counseling to patients and caregivers during illness and end-of-life care.
- Coordinate with doctors, therapists, social workers, and other healthcare professionals to ensure comprehensive care.
- Maintain accurate patient records, nursing notes, and care documentation.
- Respond promptly to changes in patient condition and report concerns to the medical team.
- Conduct home visits and follow-up assessments as required by the palliative care program.
- Ensure infection prevention and patient safety standards are maintained at all times.
- Participate in community awareness programs and palliative care initiatives when required.
- Uphold patient dignity, confidentiality, and ethical nursing practices
Pay: ₹12,000.00 - ₹25,000.00 per month
