We are looking for a compassionate, organized, and proactive Post Care Coordinator to manage the end-to-end patient journey. The Care Coordinator will act as the primary point of contact for patients, ensuring timely communication, appointment coordination, documentation, follow-ups, and overall patient satisfaction.
Essential Job Functions
- Coordinate the complete patient care journey from consultation to follow-up.
- Schedule and manage patient appointments, admissions, and procedures.
- Coordinate with patients regarding their medical prescriptions, ensuring they understand prescribed medications and assisting with prescription-related queries.
- Serve as the primary point of contact for patient queries and support.
- Conduct pre- and post-treatment follow-ups to ensure continuity of care.
- Coordinate with doctors, hospitals, diagnostic centers, and insurance partners.
- Maintain accurate patient records and update the CRM/Hospital Management System.
Pay: ₹16,000.00 - ₹18,000.00 per month
