Roles and Responsibilities:
- Home visits, visit approximately 4 to 5 enrolled patients in a day.
- To assess the patients needs and formulate personalized care plan based on comprehensive assessment
- To deliver outstanding clinical care guided by evidence based medicine and ethical practices
- To monitor the care delivered and provide the necessary clinical leadership to facilitate better recovery
- To provide Goal based care for patients resulting in better outcomes
- To implement clinical care protocols and pathways for common home healthcare presentations
- Report on number of visits and maintain the documentation of the visits.
Pay: ₹40,000.00 - ₹70,000.00 per month
