Roles & Responsibilities
1. Patient Documentation
- Type consultation notes as dictated by doctors during OPD consultation.
- Prepare and maintain patient records and case sheets accurately.
- Enter patient details correctly in hospital management software/system.
- Ensure proper filing of patient documents for future reference.
2. Registration & Data Entry
- Assist in patient registration and updating demographic details.
- Verify patient information before entering into the system.
- Maintain daily OPD patient records and visit details.
- Update follow-up patient information in the database.
3. Investigation & Reports Coordination
- Prepare laboratory, ECG, ECHO, TMT, scan, and investigation requisition forms as instructed.
- Coordinate with diagnostic departments for smooth patient flow.
- Ensure reports are attached to correct patient files.
4. Doctor Support
- Assist consultants during OPD by typing prescriptions or medical summaries where required.
- Prepare referral letters, medical certificates, and discharge-related drafts if instructed.
- Support doctors with quick retrieval of previous patient records.
5. Record Maintenance
- Maintain confidentiality of patient medical information.
- Organize OPD files and digital records systematically.
- Ensure all documents are stored properly for audit/reference purposes.
6. Communication & Coordination
- Coordinate with reception, nursing staff, pharmacy, laboratory, and billing department.
- Guide patients regarding report collection or follow-up procedures when required.
- Communicate appointment updates to concerned departments.
7. Daily Operational Duties
- Maintain daily OPD consultation list.
- Ensure accuracy in data entry with minimal typing errors.
- Report software/system issues to administration immediately.
- Complete assigned work within the required turnaround time.
Required Skills
- Good typing speed (English; regional language typing is an advantage).
- Basic computer knowledge (MS Word, Excel, hospital software).
- Attention to detail and accuracy.
- Ability to maintain patient confidentiality.
- Good communication and coordination skills.
- Ability to work under pressure during peak OPD hours.
Qualification
- Any Degree / Diploma in Computer Applications preferred.
- Experience in hospital OPD documentation preferred.
- Knowledge of medical terminology is an added advantage.
Pay: ₹10,000.00 - ₹15,000.00 per month
