We are seeking experienced and compassionate Home Health Nurses who are passionate about providing high-quality patient care outside traditional hospital settings.
Position Summary
The Home Health Nurse is responsible for providing skilled nursing services to patients in their homes. This role includes patient assessment, medication administration, wound care, chronic disease management, patient education, and coordination with physicians and family caregivers.
The nurse will travel to patient locations, deliver professional clinical care, maintain accurate documentation, and ensure patient safety at all times.
Key ResponsibilitiesPatient Assessment
- Conduct comprehensive patient assessments during home visits.
- Monitor vital signs and overall health status.
- Identify changes in patient conditions and escalate concerns appropriately.
- Develop and implement individualized care plans.
Clinical Care
- Administer medications as prescribed.
- Perform injections (IM, IV, SC) as authorized.
- Provide wound care and dressing changes.
- Manage catheters, feeding tubes, and other medical devices.
- Assist with post-operative care.
- Monitor chronic disease patients including diabetes, hypertension, COPD, and heart failure.
Home Health Services
- Elderly care and geriatric nursing.
- Palliative and end-of-life support.
- Rehabilitation support.
- Bedridden patient care.
- Stroke recovery support.
- Post-hospital discharge follow-up.
Patient & Family Education
- Educate patients and caregivers regarding treatment plans.
- Provide medication and disease management guidance.
- Teach infection prevention and home safety practices.
- Support lifestyle modifications and preventive care.
Documentation
- Maintain accurate visit notes using GIRAH Care systems.
- Record assessments, interventions, and patient outcomes.
- Complete incident reports when required.
- Ensure compliance with clinical documentation standards.
Care Coordination
- Communicate effectively with physicians and care teams.
- Coordinate laboratory tests, referrals, and follow-up visits.
- Participate in multidisciplinary case reviews.
Safety & Compliance
- Follow infection prevention and control protocols.
- Maintain patient confidentiality and HIPAA-equivalent privacy standards.
- Adhere to GIRAH Care clinical policies and SOPs.
- Report safety concerns promptly.
Required QualificationsEducation
- Diploma in General Nursing and Midwifery (GNM) OR
- Bachelor of Science in Nursing (B.Sc Nursing)
Registration
- Valid Tamil Nadu Nursing Council Registration.
- Active nursing license in good standing.
Experience
- Minimum 2 years of nursing experience.
- At least 1 year of Home Health, Community Health, ICU, Medical-Surgical, Geriatric, or Post-Acute Care experience preferred.
