Role Overview:
As a Care Coach for Chronic Care Management, you will play a crucial role in improving the well-being of patients with chronic conditions. Your primary responsibilities will include monitoring and tracking patient health progress, while offering guidance, support, and encouragement to help them adhere to their personalized care plans. By ensuring compliance with treatments, medications, and lifestyle modifications, you will contribute to better disease management and improved patient outcomes.
Responsibilities:
- Engage with patients over various touchpoints to understand their current medical status, past medical/surgical history, medications, lab results, etc.
- Identify additional patient needs—medications, labs, dietitian services, physiotherapy, etc.
- Assess patients’ social determinants and lifestyle status.
- Ensure patients provide complete medical histories and monitor adherence to care plans.
- Offer psychosocial support to help patients cope with chronic, acute, or terminal illnesses.
- Monitor patient progress and evaluate health improvements.
- Address patient queries using pre-defined FAQs.
- Educate patients about their chronic conditions, treatment options, and the importance of care plan adherence.
- Coach patients on the importance of compliance with labs, medications, and diet.
- Ensure accurate and efficient patient data management.
Eligibility Criteria:
- Front Desk Expertise in Hospitals: Proven experience handling patient interactions, appointments, and overall coordination at a hospital front desk.
OR
- Strong Communication in Telecom: A background in telecom services with excellent communication skills.
OR
- Call Center Experience in Healthcare: Hands-on experience managing patient calls within a hospital setting, ensuring prompt and efficient service.
Skills:
- Strong knowledge of medical conditions and medical terminologies.
- Effective patient medical history-taking skills.
- Coaching patients to comply with medical parameters.
- Clear and articulate communication with patients.
- Active listening skills and compassion.
- Passionate about patient care.
- Ability to engage patients in conversations on various topics.
- Grievance resolution and conflict-handling abilities.
- Competency in managing, recording, and storing patient information.
- Analytical mindset to interpret health data and trends effectively.
- Positive attitude, with a naturally pleasant disposition.
- Team player who can collaborate across departments.
Qualifications:
- Minimum bachelor’s degree.(Pharm D, B.Sc, M.Sc, Dietetics, Clinical Psychology,BA with Hospital experience....)
- Proficiency in English, Hindi and Telugu.

Pay: ₹20,000.00 - ₹25,000.00 per month

Schedule: Monday to Saturday
Language: English & Telugu are mandatory. Other regional languages are beneficial.
Location: Ability to reliably commute or plan to relocate before starting work is required.
