JOB DESCRIPTION – PHARMACIST

Location: Crescent Pediatric And Neonatology Centre
Job Summary
The Pharmacist Is Responsible For Ensuring The Safe, Accurate, And Timely Dispensing Of Medications To Inpatients And Outpatients. The Role Includes Maintaining Medication Inventory, Ensuring Compliance With Regulatory Requirements, Providing Drug Information, And Supporting Patient Safety Through Proper Medication Management.
Key Responsibilities
Medication Dispensing
- Dispense Medications Accurately Based On Valid Prescriptions.
- Verify Prescriptions For Completeness, Dosage, Drug Interactions, Allergies, And Contraindications.
- Label Medications Correctly With Dosage Instructions And Precautions.
- Ensure The Right Patient Receives The Right Medication At The Right Time.
Clinical Pharmacy Support
- Review Medication Orders For Appropriateness And Safety.
- Provide Drug Information And Medication Counseling To Patients And Caregivers.
- Clarify Prescription-Related Queries With Physicians When Required.
- Monitor And Report Adverse Drug Reactions (Adrs) And Medication Errors.
- Promote Rational Use Of Medicines.
Inventory & Store Management
- Maintain Adequate Stock Levels Of Medicines And Medical Consumables.
- Monitor Expiry Dates And Ensure First Expiry First Out (Fefo) Inventory Practices.
- Receive, Verify, And Record Pharmaceutical Supplies.
- Coordinate Procurement And Replenishment Of Stock.
- Ensure Proper Storage Conditions For Medicines, Including Temperature-Sensitive Products.
Controlled Drugs & Compliance
- Maintain Records Of Narcotic, Psychotropic, And Other Controlled Medications As Per Applicable Regulations.
- Ensure Compliance With Pharmacy Laws, Hospital Policies, And Licensing Requirements.
- Participate In Pharmacy Audits And Inspections.
- Maintain All Statutory Registers And Documentation Accurately.
Quality & Patient Safety
- Follow Hospital Medication Safety Policies.
- Report Medication Errors, Near Misses, And Adverse Events.
- Participate In Quality Improvement Initiatives.
- Ensure Cleanliness And Organization Of The Pharmacy.
Coordination & Communication
- Coordinate With Doctors, Nurses, And Other Healthcare Professionals Regarding Medication Requirements.
- Respond Promptly To Emergency Medication Requests.
- Maintain Professional Communication With Patients And Hospital Staff.
Administrative Responsibilities
- Maintain Pharmacy Records And Reports.
- Prepare Daily, Weekly, And Monthly Inventory Reports.
- Assist In Preparing Purchase Requests And Stock Reconciliation.
- Support Implementation Of Hospital Pharmacy Policies And Procedures.
Qualifications
- Diploma In Pharmacy (D.Pharm) Or Bachelor Of Pharmacy (B.Pharm)
- Valid State Pharmacy Council Registration (Mandatory)
Experience
- Minimum 1–3 Years Of Experience In A Hospital Or Retail Pharmacy.
- Fresh Registered Pharmacists May Be Considered For Junior Positions.
Skills Required
- Sound Knowledge Of Pharmacology And Medication Management.
- Knowledge Of Hospital Pharmacy Operations.
- Strong Attention To Detail And Accuracy.
- Good Communication And Patient Counseling Skills.
- Inventory And Stock Management Skills.
- Computer Proficiency And Familiarity With Pharmacy Management Software.
- Ability To Work Under Pressure And Handle Emergency Medication Requirements.
Working Conditions
- Rotational Shifts, Including Weekends, Nights, And Public Holidays As Per Hospital Requirements.
- May Be Required To Provide Emergency Pharmacy Services.
