Key Responsibilities
- Conduct patient assessments and assist in the management of diabetes, obesity, and related metabolic conditions.
- Develop and implement individualized treatment plans in collaboration with the supervising physician.
- Prescribe and adjust medications as permitted by applicable regulations and clinic protocols.
- Provide patient education on nutrition, physical activity, weight management, diabetes self-care, and lifestyle modifications.
- Monitor patient progress, review laboratory results, and adjust treatment plans as needed.
- Participate in in-person and telehealth consultations.
Care Coordination
- Collaborate with physicians, dietitians, diabetes educators, nurses, and other healthcare professionals to ensure comprehensive patient care.
- Coordinate referrals, diagnostic testing, and follow-up appointments.
- Support patients in achieving treatment goals through ongoing communication and care planning.
- Facilitate continuity of care for patients with complex metabolic health needs.
Documentation Skills
- Maintain accurate, timely, and comprehensive patient records in the Electronic Medical Record (EMR) system.
- Document patient encounters, treatment plans, medication changes, and follow-up recommendations.
- Ensure compliance with clinic policies, regulatory requirements, and quality standards.
- Review and interpret laboratory and diagnostic results, documenting findings appropriately.
