Medycare Billing Solutions is looking for a Case Manager to join our team and coordinate home health patient care.
Job Responsibilities
- Coordinate and manage patient care from admission through discharge.
- Communicate with clinicians, physicians, patients, and caregivers.
- Monitor patient visits, schedules, missed visits, and changes in patient condition.
- Follow up on physician orders and ensure proper documentation.
- Review clinical documentation for accuracy, completeness, and timely completion.
- Monitor OASIS, reassessments, recertifications, and discharge documentation.
- Maintain accurate patient records in the EMR/Kinnser system.
- Assist with hospitalizations, transitions of care, and discharge planning.
- Monitor certification, recertification, and authorization periods.
- Ensure compliance with agency policies, Medicare, CMS, HIPAA, and other applicable requirements.
- Identify and report quality or compliance concerns to management.
Preferred Qualifications
- Experience in Home Health is preferred.
- Knowledge of OASIS, patient care coordination, and EMR/Kinnser systems will be an advantage.
- Good communication and coordination skills.
- Ability to work independently and manage multiple patient cases.
Job Timing
US Time: 08:00 AM – 04:00 PM CST
India Time: 06:30 PM – 02:30 AM IST
Shift: Night Shift

Pay: ₹18,000.00 - ₹26,000.00 per month
