Job Summary
The Post-Operative Coordinator is responsible for coordinating and managing the continuum of care for patients following surgical procedures. This role ensures patients receive appropriate follow-up care, timely assessments, education, and support to promote safe recovery and improve clinical outcomes. The coordinator serves as the primary point of contact for patients, surgeons, nursing staff, and other healthcare professionals throughout the post-operative period.
Key Responsibilities Patient Care Coordination
Coordinate post-operative care plans in collaboration with surgeons, and other healthcare providers.
Schedule and manage follow-up appointments, wound assessments, and rehabilitation services.
Monitor patients' recovery progress through telephone follow-ups, or virtual consultations.
Ensure continuity of care from hospital discharge through complete recovery.
Patient Education
Provide comprehensive post-operative instructions regarding wound care, medications, activity restrictions, nutrition, and recovery expectations.
Educate patients and caregivers on recognizing signs and symptoms that require immediate medical attention.
Reinforce discharge instructions and answer patient questions regarding recovery.
Assist in developing and updating post-operative care protocols and patient education materials.
Qualifications
Bachelor's degree or Diploma in Nursing, Allied Health, Healthcare Administration, or a related healthcare field.
Minimum 0 -6 month of experience in surgical services, patient coordination, case management, or post-operative care.
Experience with electronic medical records (EMR) is preferred.

Pay: ₹16,000.00 - ₹20,000.00 per hour
