Key Responsibilities
- Care Planning: Assess patient needs to determine the appropriate next level of care (e.g., home health services, rehabilitation, or nursing facilities).
- Coordination: Work with doctors, nurses, and allied health teams to resolve same-day discharge barriers and manage inpatient flow.
- Logistics & Equipment: Arrange necessary medical equipment (e.g., oxygen, wheelchairs) and coordinate hospital transportation.
- Education & Communication: Explain discharge instructions, medication management, and diet plans to patients and their families.
- Follow-up: Schedule post-discharge appointments and assist with outpatient referrals.
Typical Qualifications
- Education: A bachelor’s degree in Nursing (BSc Nursing) or a related healthcare field is standard. Additional training or degrees in Hospital Administration may also be required.
- Experience: 1–3 years of clinical or hospital experience, ideally in case management or patient counseling.
- Skills: Strong clinical knowledge, understanding of medical terminology, and excellent communication skills.
Pay: ₹11,543.70 - ₹29,313.78 per month
