1. Review OP/IP medical records, discharge summaries, investigation reports, operative notes and other clinical documentation.
2. Assign appropriate *diagnosis and procedure codes* based on the doctor's documentation and applicable coding guidelines.
3. Ensure accurate and complete coding of diagnoses, procedures, investigations, complications and comorbidities.
4. Verify that all applicable medical services and procedures are appropriately captured for billing.
5. Identify incomplete or unclear documentation and coordinate with the concerned doctor/department for clarification.
6. Coordinate with the *billing, insurance/TPA and medical records departments* regarding coding-related issues.
7. Check patient bills for missing or incorrect charges related to coded services and report discrepancies.
8. Ensure timely completion of coding as per the hospital's prescribed *TAT*.
9. Conduct regular quality checks and rectify coding errors.
10. Assist with insurance/TPA queries, claim rejections and coding-related clarifications.
11. Maintain confidentiality of patient information and medical records.
12. Maintain coding-related reports, registers and productivity records.
13. Keep updated with changes in coding guidelines, insurance requirements and hospital policies.
14. Greet and assist patients, attendants and visitors* courteously and professionally.
15. Attend incoming telephone calls and direct them to the appropriate department/person.
16. Provide accurate information regarding *OP timings, doctors, departments, services and hospital facilities*.
17. Register new patients and update existing patient details accurately in the hospital information system.
18. Generate and verify *UHID/OP registration details* and ensure correct patient identification.
19.Coordinate doctor appointments, consultations and follow-up visits.
20. Manage the appointment schedule and inform patients about any changes or delays.
21. Guide patients to the appropriate *OP, ER, laboratory, radiology, pharmacy and other departments*.
22. Coordinate with doctors, nursing staff, billing and other departments to ensure smooth patient flow.
23. Handle patient queries and complaints professionally and escalate unresolved issues to the concerned supervisor/HOD.
24. Ensure that patient information is entered accurately into the system and maintain confidentiality.
25. Assist patients with billing-related queries and direct them to the appropriate billing personnel when required.
27. Maintain reception area, appointment registers, visitor records and other required documents.
28. Ensure proper communication and handover between shifts.
29. Follow hospital policies regarding patient privacy, confidentiality, communication, dress code and professional conduct.
30. Ensure that the reception desk is adequately staffed and attended during working hours.
31. Assist during hospital events, health camps and special OP arrangements when assigned.
32. Report any unusual incidents, patient complaints, system issues or operational concerns to the immediate supervisor.
33. Maintain a professional and welcoming environment at the reception at all times.
Pay: ₹15,000.00 - ₹18,000.00 per month
