Cancer hospitals spend billions on highly trained staff manually reading unstructured patient records including pathology reports, clinical notes, genomic panels, to power workflows like trial matching, registry curation, visit prep, and quality reporting.
We replace that manual work with task-driven AI agents that sit inside the EMR and process records with >95% accuracy, at scale, in real time.

We’re looking for a Backend-focused Software Development Engineer II (SDE2) who can design, build, and scale the core platform services that power our oncology AI agents.
This is not a “just ship tickets” role. You’ll own critical backend systems end-to-end , from our high-throughput EHR data ingestion pipelines and AI orchestration layer to the APIs that serve clinicians in real time. You’ll work closely with the AI, product, and infrastructure teams to ensure our platform remains fast, reliable, and secure as we scale across some of the world’s top cancer centers.
You’ll spend the majority of your time in the backend, building the data processing, workflow orchestration, and service infrastructure that forms the backbone of our platform. Strong Java/Spring Boot skills and a deep understanding of distributed systems are essential.
Here are examples of the kind of problems you’ll tackle:
- EHR Data Ingestion & Indexing: Build and optimize the OncoIndexer pipeline that ingests HL7, FHIR, CCDAs, PDFs, and scanned documents, transforming raw EHR data into a standardized, LLM-ready format at scale.
- AI Agent Orchestration: Design and extend the orchestration layer that coordinates our constellation of oncology-tuned LLMs (3B–72B parameters), managing multi-step agentic workflows for patient-level reasoning, trial matching, and data curation.
- Scalable Platform Services: Architect and maintain the microservices, APIs, and event-driven systems that process thousands of complex medical records daily, ensuring reliability, accuracy, and low-latency performance.
- Healthcare Integrations: Build and maintain secure integrations with hospital EMR systems, handling complex healthcare data standards, authentication, and compliance requirements.
- Design, build, and maintain scalable backend services, APIs, and data pipelines
- Own the architecture of high-throughput document processing and indexing systems
- Write clean, testable, production-grade code with clear documentation
- Participate in architecture discussions and drive technical design decisions for backend systems
- Independently debug and resolve complex issues across distributed services
- Collaborate cross-functionally with AI, product, data, engineering, and QA teams
- Set up and maintain monitoring, alerting, and observability for owned services
- Contribute to CI/CD pipelines, code reviews, and engineering best practices
- Ensure platform security, HIPAA compliance, and data integrity across all services

- 3–5 years of hands-on experience building production backend systems at scale
- Java and Spring Boot, strong proficiency is non-negotiable
- Strong understanding of data structures, algorithms, and system design fundamentals
- Deep experience with REST APIs, microservices, and distributed systems
- PostgreSQL or another relational database , SQL, query optimization, schema design, indexing
- Experience designing and operating high-throughput data processing or ETL pipelines
- Solid understanding of concurrency, caching strategies, and asynchronous processing
- Experience with Elasticsearch (indexing, querying, relevance tuning) in a search or data retrieval context
- Hands-on experience with Temporal or similar workflow orchestration frameworks
- Familiarity with healthcare data standards like HL7, FHIR, CCDA, DICOM
- Exposure to LLM serving infrastructure, prompt orchestration, or AI pipeline design
- Experience with event-driven architectures (Kafka, RabbitMQ, or similar)
- Experience with AWS / Azure, Docker / Kubernetes, and CI/CD tooling
- Familiarity with HIPAA compliance and healthcare data security best practices
