The Role
As a Home Healthcare Nurse, you will provide skilled nursing care at patients' homes. Most patients are elderly individuals managing chronic conditions, recovering after hospitalisation, or requiring regular clinical monitoring.
You will work within an assigned pod, covering a cluster of nearby societies, ensuring efficient routes and better continuity of patient care.
Key Responsibilities
- Conduct home nursing visits within the assigned pod.
- Perform prescribed clinical procedures, including:
- Injections
- IV infusions
- Catheterisation
- Ryle's tube feeding
- Nebulisation
- Other procedures as per competency and prescription
- Provide wound care and dressing, including post-operative, diabetic, and pressure-ulcer wounds.
- Record vitals and clinical observations during every visit through the YORO app.
- Manage medication schedules and educate families about dosage, side effects, and adherence.
- Monitor patients with chronic conditions such as:
- Diabetes
- Hypertension
- Cardiac conditions
- Respiratory conditions
- Post-stroke conditions
- Identify and report early signs of patient deterioration.
- Share a health summary with the family after every visit.
- Guide and supervise caregivers/GDAs on shared patients, including hygiene, positioning, transfers, and feeding.
- Respond to Emergency SOS calls within the assigned pod when on shift.
- Support YORO Health health camps and society-level vitals check activities.
- Maintain professional conduct, punctuality, documentation, and patient confidentiality.
Who We're Looking For
- GNM or B.Sc. Nursing – Mandatory
- ANM candidates with strong relevant experience may also be considered.
- Registered with the Maharashtra Nursing Council or eligible for registration.
- 0–5 years of experience; hospital, ICU, or home-care experience preferred.
- Freshers with strong clinical training may also apply.
- Confident in performing bedside procedures independently.
- Comfortable working as the primary clinician during home visits.
- Compassionate and patient with elderly patients and their families.
- Strong communication and patient-handling skills.
- Marathi/Hindi – Fluent; English – Working proficiency.
- Two-wheeler with a valid driving licence preferred.
- Punctual, presentable, and comfortable following uniform and grooming standards.
