Job Role:
- Monitor the prescriptions Analyze the Medication Errors and improve their reporting culture and reducing the medication related problems
- Review records of patients to determine the appropriateness of medication therapy
- Evaluate patient’s condition to ensure all issues are being treated
- Develop effective medication plans that minimize the risk of adverse side-effects
- Consult on dosages, medication substances etc. Monitor the correct administration of drugs
- Assess the results of pharmaceutical treatments
- Collaborate with healthcare professionals to ensure optimal patient care
- Keep accurate documentation of medication plans and patient progress
- Develop Triggers related to Medications Monitor appropriateness of antibiotics towards stewardship
- Monitor appropriateness of Prophylactic antibiotics as per the Policy
- Provide training to staffs (Both Pharmacy and Staff nurse) training regarding the Medications Maintain the Drug Formulary and New Drug Order Form
- Conduct Pharmacy and Therapeutic committee meeting every 3 months
- Develop clinical pharmacy programs according to policies and regulations Present the simple clinical audits.
- present as a papers in continuing education forums and publish in journals.
- Develop their skills with the help of clinical audits and various departmental programs Report Medication related triggers
- Co-ordinate all audits
- Frequently conduct Stock-audit, Narcotics audit, Meat Box audit, Refrigerator audit
- Pharmacology classes to be taken for nursing college students
- Monitoring and Reporting Adverse Drug Reactions to hospital and PVPi (through app) and Drug-Drug interactions, Drug-Food interactions, Therapeutic Duplications, High Risk Medications.
- Develop and Implement all SOPs related to medication therapy Involving in Medication Reconciliation process
- Maintain the Drug recall procedures Preparing Patient education Materials related to medications
- Involve in 5S activities
Pay: ₹25,000.00 - ₹35,000.00 per month
