Key Responsibilities:
- Review and validate medication orders for accuracy and appropriateness
- Monitor patient drug therapy and identify drug interactions or adverse effects
- Provide drug information and counseling to patients and healthcare staff
- Participate in ward rounds and clinical discussions
- Ensure compliance with hospital policies and regulatory standards
- Maintain proper documentation of clinical interventions
- Promote rational use of antibiotics and support antimicrobial stewardship programs
- Assist in medication safety initiatives and pharmacovigilance
Qualifications:
- B.Pharmacy / M. Pharmacy
- Registered Pharmacist (State Pharmacy Council)
- PCI Mandatory
Experience:
- 0–3 years (Hospital experience preferred)
