About the Role
Primacy In Home Care is seeking a dedicated and compassionate Support Coordinator to coordinate home care services, support clients and caregivers and ensure high-quality care delivery. The ideal candidate should possess strong communication, coordination and problem-solving skills with a passion for serving older adults and individuals requiring home-based care.
Qualifications
- Master's Degree in Social Work (MSW) – Mandatory
- Fresh graduates may apply
- Candidates with experience in home care, healthcare, elder care or community services will be given preference
- Excellent communication and interpersonal skills
- Ability to coordinate with clients, families, caregivers and healthcare professionals
- Good documentation and reporting skills
- Proficiency in MS Office and digital communication tools
Key Responsibilities
- Coordinate and monitor home care service delivery
- Conduct client follow-ups and maintain service records
- Liaise with clients, families and healthcare professionals
- Conduct client assessment, Assist in client care plan and care coordination
- Handle service inquiries and ensure client satisfaction
- Prepare reports and maintain documentation
- Support operational and quality assurance activities
Preferred Skills
- Strong organizational and time-management abilities
- Empathy and client-centered approach
- Ability to work independently and as part of a team
- Knowledge of geriatric care and home care services is an advantage
