Position Summary
The Home Care Assistant (HCA) provides non-clinical and basic personal care support to patients in their homes. The HCA assists with activities of daily living (ADLs), mobility support, companionship, safety monitoring, and household assistance while promoting patient comfort, dignity, and independence.
The Home Care Assistant works under the supervision of a Registered Nurse or Care Coordinator and follows GIRAH Care policies, procedures, and care plans.
Key Responsibilities Personal Care Assistance
- Assist patients with bathing and grooming.
- Assist with dressing and personal hygiene.
- Help with toileting and continence care.
- Assist with oral care and skin care.
- Support patients with feeding when required.
- Help maintain patient dignity and privacy.
Mobility & Transfer Assistance
- Assist patients with walking and mobility.
- Help patients move safely from bed to chair.
- Assist with wheelchair transfers.
- Support fall prevention measures.
- Report mobility concerns immediately.
Daily Living Support
- Assist with meal preparation.
- Support light household tasks.
- Help maintain a clean and safe environment.
- Assist with medication reminders (not administration unless specifically trained and authorized).
- Assist with shopping or errands when approved.
Companion Care
- Provide companionship and emotional support.
- Engage patients in conversation and activities.
- Encourage social interaction.
- Monitor changes in mood or behavior.
- Report concerns to supervisors.
Patient Observation
- Observe and report:
- Changes in physical condition
- Skin breakdown
- Falls
- Appetite changes
- Behavioral changes
- Safety concerns
- Escalate urgent concerns immediately.
Documentation
- Complete visit notes accurately.
- Record care provided during each visit.
- Maintain patient confidentiality.
- Use GIRAH Care mobile app for visit documentation.

QualificationsRequired
- Higher Secondary (12th Pass) preferred.
- Home Health Aide, Caregiver, Nursing Assistant, or equivalent certification preferred.
- Basic understanding of patient care and hygiene.
- Ability to communicate in Tamil.
- Physically fit for patient mobility assistance.
Preferred
- 6 months to 2 years of caregiving experience.
- Experience in home healthcare.
- Basic first aid knowledge.
- Additional language skills (English, Hindi, Telugu, Malayalam).
