The Medical Social Worker provides psychosocial support to patients and their families, helping them cope with the emotional, social, and financial challenges associated with illness. The role involves patient counseling, discharge planning, coordination with government health schemes, community resources, and multidisciplinary healthcare teams to ensure comprehensive patient care.
- Conduct psychosocial assessments of patients and families.
- Provide emotional support and counseling to patients and caregivers.
- Assist patients in accessing government health schemes, insurance benefits, and financial assistance programs.
- Coordinate discharge planning with clinical teams to ensure continuity of care.
- Educate patients and families regarding disease management, rehabilitation, and available community resources.
- Support vulnerable patients, including senior citizens, children, terminally ill patients, and economically disadvantaged individuals.
- Facilitate referrals to rehabilitation centers, NGOs, support groups, and community organizations.
- Work closely with doctors, nurses, psychologists, and other healthcare professionals to develop patient-centered care plans.
- Maintain accurate patient counseling records and documentation.
- Participate in patient education programs, health awareness campaigns, and community outreach initiatives.
- Ensure compliance with hospital policies, NABH standards, ethical guidelines, and patient confidentiality.
- Assist in crisis intervention, grief counseling, and bereavement support when required.
- Master's Degree in Social Work (MSW) – Medical & Psychiatric Social Work preferred.
- Bachelor's Degree in Social Work (BSW) may be considered with relevant experience.
