Role Overview
The Care Manager is responsible for ensuring the overall well-being, safety, and comfort of elderly members by providing both medical and non-medical support services. The role involves regular member interaction, emergency coordination, care planning, family communication, digital care management, and service quality monitoring to ensure seamless eldercare support.
Key Responsibilities
1. Member Care & Support
- Conduct regular home visits to assess members’ physical, emotional, and social well-being.
- Provide compassionate support and companionship to members.
- Assist members with day-to-day requirements and recreational activities such as walks, shopping, or outings.
- Offer emotional support and help members manage stress, anxiety, or loneliness.
- Ensure members receive timely and quality services as per their care plans.
2. Medical Assistance
- Accompany members to doctor consultations, hospital admissions, and follow-up visits.
- Coordinate post-hospitalization and recovery care.
- Assist members with diagnostic tests and medical appointments.
- Arrange medicine delivery and collect medical reports from diagnostic centres.
- Monitor basic health parameters/vitals when required.
- Coordinate with healthcare professionals, caregivers, nurses, physiotherapists, and diagnostic partners.
3. Non-Medical Assistance
- Support members with essential errands and daily needs including:
- Grocery and essential item delivery
- Bank, Post Office, PF, Passport, Ration, Gas Office, and other paperwork-related visits
- Accompanying members for walks, shopping, or social activities
- Ensure convenience, comfort, and safety of members during all engagements.
4. Care Planning & Assessment
- Develop and implement personalized short-term and long-term care plans.
- Conduct assessments to understand members’ evolving care needs.
- Maintain proper documentation of care plans, member history, and service updates.
5. Emergency Management
- Respond to emergencies on a priority basis and coordinate immediate support.
- Work closely with the operations team and service providers during emergencies.
- Ensure timely escalation and resolution of urgent situations.
6. Coordination & Communication
- Maintain regular communication with members and their families, especially children residing abroad.
- Provide timely updates regarding member health, services, and emergencies.
- Coordinate effectively with internal teams, caregivers, partners, and healthcare providers.
7. Digital Care Management & Operations
- Manage member onboarding through the ELDERTECH app/platform.
- Conduct virtual and physical assessments and update digital care plans.
- Monitor service delivery, attendance, and task completion through the app.
- Maintain accurate records of member visits, health updates, escalations, and service reports.
- Handle app-based escalations, complaints, and emergency coordination.
8. Partner & Service Coordination
- Assist in arranging additional caregivers or support staff when required.
- Coordinate with service partners such as nurses, physiotherapists, diagnostic centres, and medical equipment providers.
- Ensure availability and timely deployment of support services.
9. Community Outreach & Member Engagement
- Participate in local events, hospitals, and senior communities to promote services.
- Build referral relationships with doctors, hospitals, clinics, and residential communities.
- Encourage member engagement and service awareness.
Pay: Up to ₹18,000.00 per month
