Job Title: Consultant Diabetologist (Chronic Care Programmers)
Location: On-Site
Timings: 5-6 Days in a week (work from office)
Qualifications:
- MBBS with Diploma in Diabetology, Fellowship in Diabetes, CCEBDM/CCGDM or equivalent
Experience:
- Minimum 5 years of experience, 3+ years in outpatient chronic disease management
Language Proficiency:
- English (mandatory)
- Proficiency in local languages is an added advantage
Job Description:
- Conduct enrolment and baseline consultations for clinical-track members history, medication reconciliation, risk stratification, programme assignment, individualized target setting.
- Run scheduled review consultations and medication titration per programme cadence own all prescriptions and laboratory orders
- Act as the escalation point for care managers and allied clinicians; respond to urgent flags within agreed service levels.
- Review every member-reported adverse event and side effect; decide on dose change, hold or programme exit.
- EMR and Computer Proficiency: Comfortable using Electronic Medical Record (EMR) systems for patient management and documentation.
- Continuous Learning: Stay updated with the latest medical knowledge, treatment guidelines, and industry standards.
