Job Description:
- Provide individual and group diabetes education to patients and their families.
- Coordinate and facilitate diabetes support groups.
- Monitor patient progress and provide ongoing support and follow-up.
- Develop and implement diabetes education programs.
- Provide patient and family education on diabetes self-management.
- Collaborate with other healthcare professionals to ensure the best care for patients.
- Maintain accurate patient records and documentation
- Assist the Doctors in OP for documentation purposes.
