About Lifetime Health
Lifetime Health provides post-discharge care at home, delivered by qualified nurses (GNM/BSc, Karnataka Nursing Council-registered). We help patients recover safely in the weeks after they leave hospital, and flag anything concerning back to the treating team. We have supported 20,000+ families across 120,000+ home sessions, and we are now partnering with hospitals to bring this to their discharged patients.
The role
You are the face of Lifetime Health inside our partner hospitals. Based at the hospital’s discharge area in co-branded uniform, you meet the families of patients being discharged and guide them, warmly and simply, through how we will support their loved one’s recovery at home. You are the bridge between the hospital’s discharge team and our home-care nurses, making the handoff from hospital to home feel seamless, reassuring, and part of the hospital’s own care.
What you’ll do
- Be present on-site at your assigned partner hospital, in co-branded uniform, as a professional and approachable point of contact.
- Receive patient referrals from the hospital’s treating doctors and discharge team.
- Meet families at discharge; explain our post-discharge care simply and with empathy; answer questions and ease concerns.
- Understand each patient’s home-care needs and coordinate the right setup with our nursing and operations team.
- Ensure a smooth, timely handoff so care begins at home without gaps.
- Build trusting relationships with hospital staff, nursing, discharge and department teams.
- Keep accurate records of referrals and handoffs, and support the weekly report back to the hospital.
- Gather family feedback, flag issues to the operations team promptly, and protect patient privacy and dignity at all times.
What we’re looking for
- A warm, empathetic and confident communicator who can put anxious families at ease.
- Comfortable working on-site in a hospital and interacting with clinical staff.
- Fluent in Kannada and English (Hindi a strong plus), able to speak with families in their own language.
- Well-organised, reliable and punctual, with strong follow-through.
- A healthcare or patient-facing background, and a genuine, patient-first attitude.
- High integrity and sound judgement about when to escalate.
Nice to have
- A nursing qualification (GNM/BSc Nursing) or allied-health background, which helps you understand patients’ needs and speak credibly with clinical teams.
- Experience in hospital coordination, patient relations, or home-healthcare.
- Familiarity with the Bengaluru hospital ecosystem.
Pay: ₹30,000.00 - ₹37,826.66 per month
