Job Summary:
The Pharm D professional is responsible for providing clinical pharmacy services, ensuring safe and effective medication use, supporting physicians in patient care, monitoring drug therapy outcomes, and educating patients regarding medications. They work closely with doctors, nurses, and other healthcare professionals to optimize patient treatment outcomes.
Key Responsibilities:
- Review medication orders and prescriptions for appropriateness and safety.
- Conduct medication reconciliation during admission, transfer, and discharge.
- Monitor patients for adverse drug reactions (ADRs) and drug interactions.
- Participate in ward rounds with physicians and healthcare teams.
- Provide drug information and medication counseling to patients and caregivers.
- Assess medication effectiveness and recommend therapy modifications when required.
- Maintain patient medication records and clinical documentation.
- Support antimicrobial stewardship and medication safety initiatives.
- Report and document adverse drug reactions and medication errors.
- Assist in clinical audits, research activities, and quality improvement projects.
- Educate healthcare professionals on rational drug use and new medications.
Required Qualifications:
- Pharm D (Doctor of Pharmacy) from a recognized institution.
- Registration with the State Pharmacy Council.
- Freshers or experienced candidates can apply.
- Strong knowledge of pharmacology, therapeutics, and clinical pharmacy practices.
Required Skills:
- Clinical decision-making and analytical skills.
- Knowledge of drug interactions and medication safety.
- Excellent communication and patient counseling skills.
- Documentation and report-writing abilities.
- Ability to work effectively in a multidisciplinary healthcare team.
- Proficiency in MS Office and Hospital Information Systems (HIS).
Preferred Experience:
- Experience in hospital pharmacy, clinical pharmacy, pharmacovigilance, or clinical research.
- Exposure to NABH standards and medication safety practices.

Pay: ₹21,000.00 - ₹30,000.00 per month
