The Home Care Physiotherapist is responsible for providing professional physiotherapy assessment, treatment, rehabilitation, and patient education services at patients' homes. The role focuses on improving mobility, reducing pain, enhancing functional independence, and ensuring high-quality patient care in a home-based setting.
Key Responsibilities1. Patient Assessment & Treatment
- Conduct comprehensive physiotherapy assessments at patients' homes.
- Develop individualized treatment plans based on clinical findings.
- Provide evidence-based physiotherapy interventions.
- Monitor patient progress and modify treatment plans as required.
- Maintain treatment goals and outcome measures.
2. Home-Based Rehabilitation
- Deliver rehabilitation services for orthopedic, neurological, sports injury, geriatric, and post-surgical patients.
- Train patients in therapeutic exercises and functional activities.
- Educate family members and caregivers on patient care and exercise programs.
- Recommend assistive devices and home modifications when necessary.
3. Documentation
- Complete assessment forms, treatment notes, and progress reports.
- Update patient records accurately and promptly.
- Maintain confidentiality of all patient information.
- Submit daily visit reports to the clinical team.
4. Patient Education
- Educate patients on posture, ergonomics, injury prevention, and self-management strategies.
- Provide home exercise programs and follow-up instructions.
- Encourage treatment compliance and lifestyle modifications.
5. Coordination & Communication
- Coordinate with physicians, nurses, caregivers, and other healthcare professionals.
- Report patient progress and concerns to the Clinical Incharge.
- Attend clinical meetings and case discussions when required.
6. Safety & Professional Standards
- Follow infection control and patient safety protocols.
- Maintain professional conduct during home visits.
- Ensure ethical and legal standards are followed at all times.
- Carry and maintain treatment equipment responsibly.

Qualifications
- Bachelor of Physiotherapy (BPT) or Master of Physiotherapy (MPT).
- Valid professional registration/license.
- Experience in home care, rehabilitation, or clinical physiotherapy preferred.
