We are seeking a dedicated and knowledgeable Post Discharge Coordinator to support patients after hospital discharge and ensure continuity of care. The ideal candidate should have prior hospital experience, strong communication skills, and good medical knowledge to coordinate follow-ups, patient education, and recovery support.
Key Responsibilities
- Coordinate and monitor patients after discharge from the hospital.
- Conduct follow-up calls to check patient condition and recovery progress.
- Explain discharge instructions, medications, diet plans, and follow-up schedules.
- Coordinate with doctors, nurses, pharmacy, insurance, and other departments.
- Maintain accurate patient records and update reports regularly.
- Identify patient concerns or complications and escalate to the medical team when required.
- Ensure patient satisfaction and smooth continuity of care.
Requirements
- BAMS OR Bachelor’s degree in Healthcare, Nursing, Pharmacy, Life Sciences, or related field.
- Minimum 1–3 years of hospital experience preferred.
- Good understanding of medical terminology and patient care processes.
- Strong communication and coordination skills.
- Preference will be given to BAMS Candidates.
Preferred Skills
- Experience in patient counseling or care coordination.
