Job Summary
The Clinical Pharmacist is responsible for ensuring the safe, effective, and rational use of medications through direct patient care, medication therapy management, clinical consultation, and collaboration with healthcare professionals. The role involves reviewing prescriptions, monitoring drug therapy, preventing medication errors, and promoting evidence-based pharmaceutical care.
Key ResponsibilitiesClinical Services
- Review and validate medication orders for appropriateness, dosage, route, frequency, and potential interactions.
- Participate in multidisciplinary patient care rounds.
- Monitor patient medication therapy and recommend necessary interventions.
- Identify, prevent, and resolve medication-related problems.
- Provide pharmacokinetic and therapeutic drug monitoring services.
- Assess adverse drug reactions (ADRs) and report them as per hospital policy.
Patient Care
- Counsel patients and caregivers regarding medication use, side effects, precautions, and compliance.
- Educate patients on disease management and medication adherence.
- Provide discharge medication counseling when required.
Medication Safety & Quality
- Promote medication safety initiatives throughout the hospital.
- Participate in medication error reporting and root cause analysis.
- Ensure compliance with hospital medication management policies.
- Support accreditation standards and quality improvement activities.
Drug Information Services
- Provide evidence-based drug information to physicians, nurses, and other healthcare professionals.
- Assist in the development and updating of clinical guidelines, protocols, and formularies.
- Conduct medication utilization reviews and drug use evaluations.
Documentation & Reporting
- Document clinical interventions and patient counseling activities.
- Maintain accurate records related to pharmaceutical care services.
- Prepare clinical pharmacy reports and statistics as required.
Education & Training
- Conduct training sessions for healthcare professionals on medication-related topics.
- Participate in continuing professional development activities.
- Mentor pharmacy interns, trainees, and junior pharmacists.
Qualifications
- Pharm D / M.Pharm (Pharmacy Practice/Clinical Pharmacy) from a recognized institution.
- Registered Pharmacist with the State Pharmacy Council.
- Certification in clinical pharmacy practices is desirable.
Experience
- Freshers or candidates with 1–3 years of clinical pharmacy experience in a hospital setting.
- Experience in NABH/JCI-accredited hospitals will be an added advantage.

Pay: ₹15,000.00 - ₹25,000.00 per month
Work Location: In person
