BACKGROUND
The Hans Foundation, established in 2009, is a Public Charitable Trust that works for the improvement of quality of life for underprivileged communities across India through Health, Education, Livelihood and Disability sector interventions. THF undertakes direct implementation of projects on the ground in addition to providing funding support to not-for-profit organisations in India through its donor – RIST, USA. THF’s Strategic planning focuses on key areas of its work with the aim of addressing the key issues of poverty alleviation, economic inequalities, and 360° impact on the quality of life through social development programs. THF’s programs are majorly implemented in the most rural and under-developed areas in the country. Over the years, THF has expanded its charitable activities in collaboration with State and Central Governments, institutions, corporates, academia, and non-government organizations for sustainable interventions to achieve scale and economy. The interventions implemented by the foundation range from grass root level to national level programs.

JOB PURPOSE
The Social Worker will play a critical role in strengthening beneficiary support systems, ensuring follow-up compliance, facilitating access to government entitlements, and enhancing long-term rehabilitation outcomes for beneficiaries under the Hans Paediatric Program (HPP), with a primary focus on the Cochlear Implant Programme (CIP).
The role involves direct engagement with beneficiaries and families, coordination with therapy centers and government departments, documentation support, awareness building, and continuous monitoring of beneficiary progress.
KEY ACCOUNTABILITIES
1. Beneficiary Communication & Follow-up

Establish regular contact with beneficiaries and families across HPP programmes.

Act as the primary point of contact for concerns, guidance, and support.

Conduct telephonic follow-ups with ~1,100 cochlear implantees (surgery since 2013).
-
Maintain detailed call logs, interaction records, and follow-up reports.
-
Coordinate with therapy centers and parents to strengthen rehabilitation tracking.

CI beneficiaries will be connected to MTB and CDC centre for the Therapy and follow-up, as per needed.
2. Documentation & Verification
-
Ensure beneficiaries have required documents: disability certificates, UDID cards, medical records.

Guide families through documentation for government schemes and welfare programmes.

Maintain updated beneficiary databases for reporting, compliance, and monitoring.
-
Track disability certification, therapy compliance, rehabilitation progress, and educational support.
3. Government Linkages & Scheme Facilitation
-
Facilitate access to disability pensions, scholarships, and educational support schemes.
-
Coordinate with district disability offices, hospitals, and local authorities.

Target: 90% of cochlear implantees from April 2023 to till March 2026 without UDID cards obtain them within the year.

Target: up to 60% enrollment of eligible beneficiaries under disability pension schemes.
4. Family Counseling & Guidance

Provide counseling on rehabilitation, therapy compliance, and government entitlements.

Conduct awareness sessions on disability rights, welfare schemes, and rehabilitation.

Develop simple informational materials such as FAQs and guidance checklists for parents.
5. Monitoring, Reporting & Compliance
-
Maintain comprehensive beneficiary tracking systems and monitoring tools.

Monitor rehabilitation compliance, therapy attendance, and communication outcomes.

Prepare monthly progress reports with achievements, challenges, and corrective actions.

Recommend process improvements based on field observations.
6. Problem Resolution & Escalation
-
Identify beneficiaries facing barriers to government schemes or therapy services.
-
Escalate unresolved cases for higher-level intervention as required.
-
Recommend policy improvements based on beneficiary feedback.
7. Community & Institutional Linkages
-
Build relationships with NGOs, disability associations, schools, and therapy centers.
-
Explore partnerships for rehabilitation, vocational training, and inclusive education.
-
Strengthen coordination between families, healthcare providers, and stakeholders.
Other Indicative Requirements
Educational Qualifications
-
Master's / Bachelor's degree in Social Work (MSW/BSW), Sociology, Psychology, or related field
Functional/Technical Skills and Relevant Experience & Other Requirements (Behavioural, Language, Certifications etc.
- Minimum 1–3 years in healthcare, disability support, community outreach, or social development.

Experience with children with disabilities preferred. Knowledge of government disability schemes desirable.

Proficiency in MS Office and database management
