Key Responsibilities
- Provide professional nursing care to patients in their homes according to physician orders and company policies.
- Conduct initial and ongoing patient assessments, including physical, emotional, and environmental evaluations.
- Develop, implement, and update individualized care plans.
- Administer medications, injections, IV therapy, wound care, and other prescribed treatments.
- Monitor patient health status and document observations, interventions, and outcomes accurately.
- Educate patients and family members on disease management, medication administration, nutrition, and preventive care.
- Assist patients with activities of daily living when required.
- Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
- Respond promptly to changes in patient condition and emergencies.
- Maintain infection control standards and patient safety protocols.
- Ensure compliance with healthcare regulations, professional standards, and company policies.
- Participate in quality improvement initiatives and staff training programs.
Qualifications
- Diploma, B.Sc. Nursing, or equivalent nursing qualification from a recognized institution.
- Valid nursing registration/licensure with the relevant nursing council.
- Minimum 1–3 years of clinical nursing experience (home care experience preferred).
- Knowledge of patient assessment, medication administration, wound care, and chronic disease management.
- Current certification in Basic Life Support (BLS); Advanced Cardiac Life Support (ACLS) is an advantage.
- Ability to work independently and make sound clinical decisions.
- Strong interpersonal, communication, and documentation skills.
- Willingness to travel to patient homes as required.

Working Conditions
- Provide care in patients' homes across assigned service areas.
- Daily 12 hour working and on-call duties as needed.
- Frequent travel between patient locations.
