At Ayu Devices, we've built a comprehensive, data-driven, scientific evidence-based program to help patients manage and reverse these conditions - combining continuous glucose monitoring (CGM), wearable health bands, personalized nutrition, structured strength training, and dedicated human support, all built around a real onboarding journey rather than a generic app.
About the Role
We're looking for a Care Coordinator to be the human backbone of a patient's journey with us from the day they onboard through their entire program. This is not a call-center role. You will own the end-to-end relationship with a panel of patients: understanding their comprehensive bloodwork, following their personalized diet plan, tracking their CGM and health band data, and being the one consistent, trusted voice guiding them daily toward better health.
Given the clinical depth of this role (interpreting diet-CGM correlations, guiding exercise safely, understanding musculoskeletal and metabolic health basics), we require a Physiotherapy background (BPT/MPT).
What You'll Do
Patient Onboarding & Journey Ownership
● Own a panel of patients end-to-end - from onboarding through active program management
● Review each patient's comprehensive bloodwork and onboarding data to understand their baseline health status
● Coordinate CGM patch and health band fitting/setup, and ensure patients understand how to use their devices
Daily Patient Engagement
● Call each patient a minimum of 3 times daily to:
○ Log and review food intake against their personalized diet plan
○ Confirm medication adherence and flag any issues or side effects
○ Check in on how they're feeling, energy levels, and any symptoms
● Actively manage patient WhatsApp groups — answering questions, sharing reminders, and being visibly present and responsive throughout the day.
Strength Training Adherence
● Ensure every patient joins their evening strength training session
● Follow up with non-attendees same-day to understand barriers and re-motivate
● Track attendance and flag consistent drop-off patterns to the clinical team.
Clinical Coordination
● Correlate patient-reported food/lifestyle data with CGM trends and flag significant deviations to the clinical/nutrition team
● Escalate any red-flag symptoms (hypoglycemia, BP spikes, etc.) immediately per protocol ● Maintain accurate, up-to-date records of every patient interaction.
Patient Relationship & Retention
● Build genuine trust and rapport — patients should feel cared for, not monitored
● Proactively address concerns, confusion, or drop in motivation before they become a reason to disengage from the program.
What We're Looking For
Must-Have
● Bachelor's or Master's degree in Physiotherapy (BPT/MPT)
● Strong verbal communication skills in English and Kannada/Hindi (additional regional languages a plus)
● Comfortable with high call volume — this role involves structured, repetitive daily outreach at scale
● Basic understanding of diabetes, hypertension, nutrition, and exercise physiology
● Comfortable using WhatsApp, mobile apps, and basic dashboards/CRM tools for patient tracking
● Empathetic, patient, and consistent — this role is built on relationship and trust, not transactions
Good to Have
● Prior experience in patient counseling, tele-health, chronic disease management, or a clinical call-center-style role
● Familiarity with CGM devices, fitness wearables, or remote patient monitoring platforms
● Experience working with structured diet plans or exercise programs

Pay: ₹25,000.00 - ₹35,000.00 per month
