Job Description
A Discharge Summary Operator is responsible for preparing, typing, reviewing, and maintaining patient discharge summaries and related medical records accurately and within the required turnaround time. The role involves coordination with doctors, nurses, and medical records departments to ensure proper clinical documentation and smooth patient discharge processes.
Key Responsibilities
- Prepare and type patient discharge summaries from doctors’ notes or dictation.
- Ensure accuracy of medical terminology, diagnosis, treatment details, and discharge instructions.
- Coordinate with consultants, nursing staff, and medical records department.
- Maintain patient confidentiality and hospital documentation standards.
- Review and proofread discharge summaries before final submission.
- Update patient information in EMR/HIS software systems.
- Complete discharge summaries within specified timelines.
- Maintain records of pending and completed summaries.
- Assist in documentation audits and quality improvement activities.

Qualification
Experience
- 2-7 years of experience in hospital discharge summary preparation or medical transcription.
Salary
- ₹15,000 – ₹30,000 per month (depending on experience and hospital standards).
Pay: ₹15,000.00 - ₹30,000.00 per month
