Responsibilities:

- Update patient history, examination details, and treatment plans in EMR/EHR systems
- Ensure accuracy and completeness of medical documentation
- Maintain confidentiality and HIPAA compliance
- Coordinate with healthcare providers for documentation support
Requirements:
- 5–6 years of experience as a Medical Scribe or Medical Documentation Specialist
- Strong knowledge of medical terminology and clinical workflows
- Experience with EMR/EHR systems
- Excellent typing speed and attention to detail
- Good communication and listening skills
- Ability to work in rotational/night shifts if required
