Key Responsibilities
- Review and validate medication orders and prescriptions for appropriateness, safety, dosage, interactions, contraindications, and allergies.
- Participate in patient medication history and medication reconciliation.
- Monitor patients for adverse drug reactions, drug interactions, therapeutic response, and medication-related problems.
- Provide evidence-based recommendations to physicians regarding drug selection, dosage, route, and duration of therapy.
- Participate in clinical rounds with doctors and other healthcare professionals where applicable.
- Provide counselling to patients and caregivers regarding medicines, dosage, administration, precautions, and possible side effects.
- Monitor and document adverse drug reactions and medication errors according to hospital policy.
- Support antimicrobial stewardship and rational use of medicines.
- Review laboratory and clinical parameters relevant to medication therapy, including renal and hepatic function where appropriate.
- Ensure compliance with hospital formulary, medication-use policies, and applicable regulatory requirements.
- Assist in developing and updating medication protocols, guidelines, and formulary information.
- Provide drug information and evidence-based medication advice to healthcare professionals.
- Participate in medication safety initiatives, audits, quality improvement activities, and clinical pharmacy programs.
- Maintain accurate clinical documentation and pharmacy records.
- Coordinate with nursing, medical, laboratory, and other departments to ensure continuity of pharmaceutical care.
- Support training and education of pharmacy students, interns, nurses, and other healthcare staff when required.
- Perform other duties assigned by the Pharmacy Manager or hospital management.
Pay: ₹30,000.00 - ₹35,000.00 per month
