Job Title: Community Coordinator
Reporting to: Program Coordinator
Job Purpose:
To facilitate the community-based rehabilitation (CBR) of persons with spinal cord injury (PwSCI) by coordinating identification, goal-setting, referrals, service delivery, stakeholder engagement, and documentation.
Key Responsibilities:
1. Conduct functional assessments and provide therapy to PwSCI through home visits, camps, and tele-rehabilitation. Refer cases to specialized rehabilitation centers and coordinate follow-up care.
2. Work closely with PHCs, hospitals, and local service providers to strengthen referral systems and ensure availability of self-care kits, medical support, and assistive devices.
3. Organize training sessions for parents, caregivers, and local stakeholders. Lead sensitization programs and support the formation of peer support groups or district-level federations.
4. Facilitate beneficiary access to government entitlements such as UDID, social security schemes, and livelihood support. Mobilize local resources to support rehabilitation and sustainability.

Maintain accurate records of services and beneficiary progress using project systems.
Key Deliverables:

Timely and accurate documentation of activities and beneficiary data.

Strong coordination with stakeholders (PHCs, hospitals, local government).

Improved rehabilitation outcomes and community integration for PwSCI.

Reports on community engagement, training, and resource mobilization.
Qualifications and Experience:

Completed CCCG /CCRT course certified by RCI, /Bachelor’s degree in Social Work, Any degree or related field.

Experience in community development, rehabilitation, or health sector preferred, freshers can also apply.

Understanding of disability rights, inclusive development, and government schemes.

Familiarity with documentation tools and digital platforms.
Preferred Skills:

Prior experience working with persons with disabilities or SCI.

Proficiency in local language, MS Office, and mobile communication apps.

Strong interpersonal skills and willingness to travel to field sites regularly.
