Position Summary
GIRAH Care is seeking a qualified and compassionate Certified Pharmacist to oversee medication dispensing, ensure regulatory compliance, support home healthcare operations, and provide medication counseling to patients and caregivers.
The Pharmacist will play a critical role in ensuring safe, accurate, and timely delivery of medicines to patients receiving care at home.
Key ResponsibilitiesClinical & Pharmacy Operations
- Review and validate prescriptions received from physicians.
- Dispense medications accurately in accordance with applicable laws and regulations.
- Verify dosage, frequency, route, and potential drug interactions.
- Ensure proper labeling, packaging, and storage of medicines.
- Maintain inventory of medicines, consumables, and medical supplies.
- Monitor expiry dates and ensure timely removal of expired products.
- Coordinate medicine procurement from authorized distributors and manufacturers.
- Support medication reconciliation for home healthcare patients.
Patient Care
- Educate patients and caregivers on:
- Medication usage
- Dosage schedules
- Storage requirements
- Potential side effects
- Drug interactions
- Counsel patients on chronic disease medications including:
- Diabetes
- Hypertension
- Cardiac conditions
- Respiratory diseases
- Respond to patient medication-related inquiries.
Home Healthcare Support
- Collaborate with physicians, nurses, and care coordinators.
- Prepare medication kits for home visits.
- Support nursing staff with medication administration protocols.
- Assist in developing medication adherence programs.
- Participate in medication error prevention initiatives.
Regulatory & Compliance
- Ensure compliance with:
- Drugs and Cosmetics Act, 1940
- Pharmacy Act, 1948
- State Pharmacy Council regulations
- Applicable NABH standards
- Maintain all statutory records and registers.
- Assist with pharmacy licensing and inspections.
- Ensure controlled drugs are managed as per legal requirements.
- Maintain confidentiality and patient privacy in accordance with company policies.
Technology & Documentation
- Use pharmacy management systems and electronic medical records (EMR).
- Maintain accurate dispensing and inventory records.
- Generate reports related to inventory, prescriptions, and usage patterns.
- Document medication incidents and adverse drug reactions.
QualificationsRequired
- B.Pharm or D.Pharm from a recognized institution.
- Must possess a valid Registered Pharmacist License issued by the State Pharmacy Council.
- Registration with Tamil Nadu Pharmacy Council preferred.
- Minimum 1-3 years of pharmacy experience.
Preferred
- Experience in:
- Retail pharmacy
- Hospital pharmacy
- Home healthcare pharmacy
- Online pharmacy operations
- Knowledge of chronic disease medications and home care protocols.
Skills & Competencies
- Strong knowledge of pharmacology and medication management.
- Excellent communication and counseling skills.
- Thorough understanding of pharmacy laws and regulations.
- Inventory management skills.
- Computer proficiency (MS Office, pharmacy software, EMR systems).
- Attention to detail and high level of accuracy.
- Ability to work independently and in multidisciplinary teams.
- Strong ethical and professional standards.
Certifications
Candidates must possess:
- Valid State Pharmacy Council Registration.
- Registered Pharmacist Certificate.
- Current professional license in good standing.
Preferred certifications:
- Basic Life Support (BLS)
- Good Pharmacy Practice (GPP) Training
- NABH Awareness Certification
Working Conditions
- Office-based and field support activities.
- May require occasional travel for partner pharmacy audits and operational support.
- Rotational shifts, weekends, and holiday duties may be required.
