We are looking for a passionate and dedicated Clinical Pharmacist to join our Clinical Pharmacy team and contribute towards improving medication safety and patient outcomes.

Qualification
- Pharm.D / M.Pharm (Pharmacy Practice / Clinical Pharmacy)
- Registration with the State Pharmacy Council is mandatory.
Job Location

Key Responsibilities
- Participate in multidisciplinary ward rounds and collaborate with physicians, nurses, and other healthcare professionals.
- Review medication prescriptions for appropriateness, safety, dosage, drug interactions, and therapeutic effectiveness.
- Identify, document, and report medication-related problems and adverse drug reactions (ADRs).
- Provide evidence-based drug information and therapeutic recommendations to healthcare professionals.
- Counsel patients and caregivers regarding medication use, dosage, side effects, storage, and adherence.
- Perform medication reconciliation during admission, transfer, and discharge.
- Monitor antimicrobial usage and support antimicrobial stewardship initiatives.
- Ensure compliance with hospital policies, NABH standards, and medication safety protocols.
- Assist in preparing clinical guidelines, protocols, and medication use evaluations.
- Document all clinical pharmacy interventions accurately in patient records.
- Participate in quality improvement initiatives, audits, and continuing education programs.
Required Skills
- Strong clinical and pharmacological knowledge.
- Good understanding of medication safety principles.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- Strong analytical and problem-solving skills.
- Proficiency in MS Office and Hospital Information Systems (HIS) is preferred.
- Good documentation and presentation skills.
Preferred Competencies
- Knowledge of antimicrobial stewardship programs.
- Experience in medication reconciliation and clinical interventions.
- Familiarity with NABH accreditation standards and quality indicators.
- Commitment to patient safety and continuous professional development.
Pay: ₹12,940.69 - ₹30,000.00 per month
