Key Responsibilities:
Nutritional Assessment: Conduct comprehensive assessments of patients' nutritional status through screenings, lab results, and medical history to identify risks.
Medical Nutrition Therapy (MNT): Develop, implement, and document individualized nutritional care plans, including specialized diets, enteral (feeding tube), or intravenous nutrition.
Patient Counseling: Educate patients and their families on dietary restrictions, therapeutic diets, and lifestyle changes to manage chronic diseases.
Interdisciplinary Collaboration: Collaborate with doctors, nurses, and pharmacists to incorporate nutrition into the overall treatment plan.
Documentation & Monitoring: Maintain detailed patient records (EHR) and monitor progress to make adjustments, including food safety and quality audits
