Job Description Assess clients’ social, emotional, financial, and environmental needs
- Develop and implement individualized care or intervention plans
- Provide counseling, guidance, and crisis intervention services
- Connect clients with community resources, benefits, and support programs
- Advocate for clients’ rights and access to services
- Maintain accurate case records and documentation
- Collaborate with healthcare providers, educators, legal professionals, and community agencies
- Conduct home visits and community outreach when required
- Monitor client progress and adjust care plans as needed
- Ensure compliance with ethical standards, policies, and relevant laws
RequirementsQualifications
MSW
- Knowledge of social work ethics, safeguarding, and community resources
- Strong communication, empathy, and case management skills
- Ability to maintain accurate, confidential records
- Willingness to conduct fieldwork/home visits
- Background and reference checks as required
