The ICU Incharge is responsible for the overall administration, supervision, and smooth functioning of the Intensive Care Unit (ICU). The ICU Incharge ensures quality patient care, infection control, staff supervision, equipment maintenance, documentation, and compliance with hospital policies.
1. GENERAL RESPONSIBILITIES
- Ensure 24×7 safe and effective patient care in the ICU.
- Supervise all ICU nursing staff and support staff.
- Coordinate with ICU Consultants, Duty Medical Officers, Specialists, and other departments.
- Maintain ICU discipline and professional work culture.
- Ensure compliance with hospital policies, NABH standards, and infection control guidelines.
- Conduct shift handovers effectively.
- Maintain confidentiality of patient information.
2. PATIENT CARE RESPONSIBILITIES
- Monitor critically ill patients continuously.
- Ensure timely administration of medications.
- Verify doctor's orders before implementation.
- Monitor and document vital signs accurately.
- Ensure proper ventilator care and monitoring.
- Monitor patients receiving oxygen therapy.
- Supervise IV infusions, syringe pumps, and infusion pumps.
- Ensure timely blood sample collection.
- Coordinate laboratory investigations.
- Prepare patients for procedures.
- Assist doctors during emergency procedures.
- Monitor intake and output chart.
- Maintain nursing assessment records.
- Ensure proper pain management.
- Monitor neurological status using Glasgow Coma Scale (GCS).
- Prevent pressure ulcers.
- Ensure proper positioning of critically ill patients.
3. EMERGENCY MANAGEMENT
- Respond immediately to Code Blue emergencies.
- Coordinate CPR and Advanced Cardiac Life Support (ACLS).
- Prepare emergency medications.
- Ensure crash cart readiness.
- Verify emergency equipment functionality.
- Coordinate patient transfer to Operation Theatre or Cath Lab.
- Maintain emergency documentation.
4. STAFF MANAGEMENT
- Prepare ICU duty roster.
- Allocate patient assignments according to staff competency.
- Monitor staff attendance.
- Conduct shift handover.
- Supervise newly joined nurses.
- Train ICU staff regularly.
- Evaluate staff performance.
- Counsel staff regarding discipline.
- Report staff shortages to Nursing Superintendent/Management.
- Approve leave after arranging replacement.
5. INFECTION CONTROL
- Ensure strict hand hygiene compliance.
- Monitor PPE usage.
- Ensure isolation precautions.
- Supervise biomedical waste segregation.
- Ensure proper sterilization practices.
- Prevent Hospital Acquired Infections (HAI).
- Monitor central line care bundles.
- Monitor ventilator-associated pneumonia (VAP) prevention bundle.
- Ensure catheter-associated urinary tract infection (CAUTI) prevention.
- Conduct infection control audits.
6. EQUIPMENT MANAGEMENT
- Ensure proper functioning of:
- Ventilators
- Cardiac monitors
- Defibrillators
- ECG machine
- Syringe pumps
- Infusion pumps
- Suction apparatus
- Oxygen flow meters
- Nebulizers
- ABG machine (if available)
- Report equipment malfunction immediately.
- Coordinate preventive maintenance.
- Maintain equipment service records.
7. MEDICATION MANAGEMENT
- Check emergency drug stock daily.
- Monitor expiry dates.
- Maintain narcotic drug register.
- Verify high-alert medications.
- Ensure LASA (Look-Alike Sound-Alike) medications are properly labeled.
- Coordinate with pharmacy for medicine availability.
- Ensure proper medication storage.

10. PATIENT SAFETY

- Coordinate with Emergency Department, Operation Theatre, Laboratory, Radiology, Pharmacy, and Blood Bank.
- Guide patient relatives politely regarding ICU visiting policy.
- Escalate critical issues to hospital management.
- Maintain professional communication with all departments.

- Recommend disciplinary action.
- Verify ICU documentation.
- Ensure compliance with ICU protocols.
- Coordinate emergency response.
- Escalate operational issues to the Nursing Superintendent or Hospital Management.
